I know we ran into this using Debian/3.0

If I recall, a symlink on the 'domain' directory, i.e. 

/var/www/domain.com -> /home/httpd/username/domain.com

will cause that, but, 

/var/www/domain.com/index.html -> /var/www/domain.com/index.whatever

will work.  

If I recall, it was only when the symlink was the root of the domain.
