[PATCH 0/5] fix some tcm_vhost issues

To make tcm_vhost work again, the patchset fix some tcm_vhost issues to upgrate tcm_vhost implementation.

Todo list:
      will add some tmf functions in tcm_vhost based on latest virtio_scsi specs.

My git tree:
1.) qemu vhost integration:
    git@xxxxxxxxxx:wuzhy/qemu.git vhost-scsi-integrate
    It will rebase stefan's vhost code into qemu.git/master

2.) tcm_vhost:
    git@xxxxxxxxxx:wuzhy/linux.git tcm_vhost
