This must be some typo, it should read
<form method="post" action="' . $_SERVER['PHP_SELF'] .'">

Try using
$subject = isset($_POST['subject']) ? $_POST['subject'] : "";
$subject = isset($_POST['elvismail']) ? $_POST['elvismail'] : "";
