I suspect that what happened is that the remote SB800 did not fire. 

You might set up the shot again and watch the SB800 closely to see if it does fire.

The SB800 was set to Remote, Channel 1, Group A.

The flash control for built-in flash of the D300 was set to Commander mode, Built-in flash --, Group A, Compensation 0, Channel 1.

Below is the EXIF data. Two entries seems to indicate the cause of the problem:

"Light Source = unknown"
"Flash = Flash fired, compulsory flash mode, return light not detected".


What do these two entries mean?
