Dan,

I hate to say it but, you ain't going to find good on-demand printing. 
Double hit, B/W is very expensive. A friend pays about $3.00 ea. for a
one-page brochure advertising his edition of prints.  What about an
edition of ink-jet prints in a slip-case?  I and my friends have
explored home-made art book printing quite a lot and not found an easy
solution. My Lookaround book is laserjet printed, 8-1/2 x 11 with six,
11 x 17 B/W ink jet fold outs. I put about $5.00 ea. into materials
including padded shipping envelope.  I have also tipped (glued in) 
prints into blank, bound  books and wire-bound artist sketch books. 
The latter is the most workable.
