Opal SIP stack I understand supports ZRTP library. For non-commercial open source applications like ekiga I understand it is free however you will need a license for commercial development. For H.323 there is a commercial binary add-on library which uses OpenSSL and is capable of encrypting audio/video. PacPhone www.pacphone.com supports it.
