[RESEND PATCH v6 0/3] usb: multi-phy support

Patch series modifies the otg library to add support for multiple phys and
to get a phy by type. This was initally developed to add support for
omap5 which has both usb2 phy and usb3 phy.

It also does some cleanup like renaming _transceiver with _phy and
usb_set_transceiver is replaced with usb_add_phy to make it similar to
other usb standard function names like usb_add_hcd.

Changes from v2:
Added a patch to introduce devm_usb_(get|put)_phy() API's in order to
associate a device with usb_phy while get of the phy and removing the
association while put of the phy(devm_usb_put_phy). With this the driver
need not care about releasing the phy during driver detach.

Added an internal function called __usb_find_phy() that finds and returns
the phy as suggested by Felipe on v1 patch series.
