I'll read this again and see if it's related, thanks to reminding me.
Althogh in my case the situation is reversed: the TD is on the done
list but not in the hash, one can lead to another.

It was a completely incoherent message, I don't know how to react.
It may be possible to write to Mr. Chen and try to get more info,
but right now a collegue found a way to reproduce my issue with OHCI,
so it seems more promising to concentrate on pursuing that directly.
