There are a couple of options.

jackeq and jamin have a jack connections menu
patchage allows you to make jack connections too
An addition ardour also has a connections manager which Paul must have
forgot about briefly :-)

Are you looking for a connections app that will respond to a screen reader?

That seems like a reasonable request and it probably wouldn't take long to
take that functionality out of jamin or jackeq and make it into a stand
alone app.
