an 11c11040 agrsm chipset which appears somewhat contradictory.  The
modem is different and more information may be needed to determine if
a driver exists for this modem.  Try to send us more information like
output from dmesg as the following line explains :

 The PCI slot 00:16.0 of the modem card may be disabled early in
 a bootup process,  but then enabled later. If modem drivers load
 but the  modem is not responsive, read DOCs/Bootup.txt about possible fixes.
 Send dmesg.txt along with ModemData.txt to discuss@xxxxxxxxxxxxx
 if help is needed.

It is hard to determine a driver from the given information.
