I made an interesting revelation recently and have been testing for a while with good success, though the solution is another problem for us due to the purpose of the box. We use reiserfs on the iscsi device and this problem goes away if I disable filesystem quotas (quotaoff or just never quotaon). I had accidentally left quotas disabled for a few days at one point in time and then discovered that when I tried to turn them back on, the system failed almost immediately.
I then left quotas off for several weeks and the system has not had a single issue since then. If anyone has any advice on why this might occur or how to fix it, I would appreciate it.
