You don't drag the title bar of the palette. Instead, you drag the 
little tab that has the palette's name on it.

You can drag it into the gray part of the Options bar (the "palette 
well"), or you can drag it into the gray bar below the title bar of 
another palette to nest it with that palette.

If you drag the title bar instead, the palettes do indeed just sit on 
top of the options bar.

You may want to check that your layers palette isn't nested with 
another palette. That could cause it to seem to appear and disappear, 
and I can't think of anything else that would do that. I've been 
using 6.0 since pre-alpha, and no-one has ever seen the disappearing 
palette behavior you described in any alpha or beta version.
