Many of the answers you pose you may already know.  Be careful accepting 
yes or no's as answers.  For some of these answers represent the 
masses.  Yes there are solutions that have proven themselves, such as 
grouping by subject etc....but.  Instead of 1 8x10 per page maybe a 6x6 
(who says bigger is better) centered on an 8x10 background for example.  
Go ahead and use them H and V,  if they are small like I mentioned it 
isn't an issue, so what if everything isn't identical.   ID your 
photos?  Sure let your work do the talking, but I have seen some very 
nice books using a small discreet  logo or your name placed simply & 
strategically. I have left portfolios because  sometime it's convenient 
for the Client and regarding them copying things while they have it, 
that's a possibility, but it seems if everything has to be guaranteed 
you may as well not leave home.  Black sheets or transparent?  What is 
everyone else in your area doing-then do something different, how 'bout 
white.  Go ahead and show published work, this speaks to several things, 
you've been published, who you have worked for, and how you shot for 
layout or concept.  I have done this, where I re-photographed several 
published pieces, tweaked them in Photoshop and the printed them back 
out as I would the other images.  Don't let yourself get stuck trying to 
do it the "right way" after all your way may be the better way.

And keep in mind bad ideas are only bad if the Art Buyer thinks so.
